RESUMO

Objective: To investigate the clinicopathological features and immunohistochemical phenotypes of gastric SMARCA4-deficient undifferentiated carcinoma, and to discuss the daily diagnostics of this entity and analyze its prognosis. Methods: The cases of gastric SMARCA4-deficient undifferentiated carcinoma diagnosed at the Department of Pathology, Peking University Cancer Hospital, China from January 2010 to August 2022 were collected. The histological sections were reviewed, the immunohistochemical results and clinicopathological features were analyzed, and relevant literature was reviewed. Results: Pure foci of undifferentiated carcinoma were seen in 7 cases, and 1 case was accompanied by a moderately differentiated tubular adenocarcinoma component. Undifferentiated carcinoma foci showed similar sheet-like or solid diffuse growth pattern, medium-sized tumor cells characterized by 1-2 nucleoli, and abundant cytoplasm and rhabdoid appearance. The average patient age was 65±8 years. Six patients were male and 2 were female. Immunohistochemical staining showed that undifferentiated carcinoma of all 8 tumors were negative for SMARCA4 (BRG1). Among 7 patients who underwent SMARCA2 (BRM) and SMARCB1 (INI1) staining, 4 cases showed loss of BRM expression, 2 cases showed weakly positive staining, and 1 case was diffusely positive, but all 7 cases were diffusely strong positive for INI1. The neuroendocrine marker, synaptophysin, was weakly positive in 5 cases, while CgA and CD56 were negative in 8 cases. Ki-67 index was more than 70%. Two cases were mismatch repair deficient and showed the loss of MLH1/PMS2 expression, while 1 case showed only MSH2 loss. PD-L1 staining showed that combined positive score (CPS)≥1 in 4 cases (CPS ranging from 1 to 55) and CPS<1 in the other 3 cases. Four patients had clinical stage Ⅳ disease. Two of them died within 3 months after diagnosis. Conclusions: Gastric SMARCA4-deficient undifferentiated carcinoma/rhabdoid carcinoma is a rare group of highly malignant tumors with a poor prognosis. Loss of the core subunit of SWI/SNF complex may be associated with the development of dedifferentiated histological pattern and aggressive tumor progression, which may be more frequently accompanied with mismatch repair deficiency.

Objective@#To understand current situation and influencing factors of school bullying among left-behind and non left-behind students in vocational colleges in western Hunan, and to provide reference for the prevention and control of campus bullying in vocational colleges in poverty-stricken areas.@*Methods@#A multi-stage stratified random cluster sampling method was used to select 1 241 students from higher vocational colleges in Xiangxi for questionnaire survey. The survey included basic demographic characteristics, health-related behaviors and school bullying. Associcted factors were compared among students with and without school bullying by using the χ2 test and Logistic regression.@*Results@#The reported rate of school bullying in vocational colleges was 41.82%. The reported rate of left-behind students (45.05%) was higher than that of non-left-behind students (39.21%). The difference was statistically significant (χ2=4.29,P<0.05). Multivariate unconditional Logistic regression showed that male students and good parental relationship negatively associated with school bullying among left-behind students(OR=0.55,0.47). While rural students, 5 years of senior vocational, smoking, drinking, and game addiction were positively associated with school bullying among left-behind students(OR=1.93,2.57,2.51,3.95,4.73); good relationship with mothers was associated with less school bullying(OR=0.40), while smoke, drinking and game addiction was associated with more school bullying among non left-behind students(OR=1.86,2.32,3.81)(P<0.05).@*Conclusion@#School bullying among students in higher vocational colleges in western Hunan is highly prevalent. The reporting rate among left-behind students is higher than that of non-left-behind students. Due to different factors, joint intervention measures by government, schools and families should be carried out as soon as possible.

RESUMO

<p><b>OBJECTIVE</b>To define and evaluate the malaria transmission potential in the Yangtze River, following construction of the Three Gorges Reservoir.</p><p><b>METHODS</b>Six villages, namely, Kaixian, Fengjie, Wanzhou, Fuling, Yubei, and Zigui were selected for investigating the malaria transmission potential in the reservoir. Transmission potential was estimated by mathematical modeling and evaluation of the local malaria situation. Factors that influenced the transmission potential were analyzed using Poisson regression analysis in combination with Grey Systematic Theory (based on evaluation by the Delphi method).</p><p><b>RESULTS</b>Indirect fluorescent antibody data and the incidence of malaria in the local population were consistent with the malaria transmission potential calculated for the area. Multivariate Poisson regression analysis showed a statistically significant association between the riparian zone and the man-biting rate.</p><p><b>CONCLUSION</b>The risk of a malaria epidemic can be forecasted using the malaria transmission potential parameters investigated here which was closely correlated with the riparian zone.</p>

<p><b>OBJECTIVE</b>To investigate the histopathological features, complications, diagnosis, and differential diagnosis of coal workers' pneumoconiosis (CWP).</p><p><b>METHODS</b>The lung tissue sections from 14 autopsy cases of CWP were subjected to HE staining and observed under a light microscope, and a retrospective analysis was performed considering the occupational history and clinical features.</p><p><b>RESULTS</b>The 14 cases were 46-71 years of age (mean, 57.7 years). Two cases were diagnosed as dust reaction, 1 case as simple CWP (stage I anthracosilicosis), and 11 cases as complicated CWP (9 cases of stage II anthracosilicosis, 1 case of stage III anthracosilicosis, and 1 case of stage III silicosis). Twelve cases were complicated by chronic bronchitis and emphysema, 8 cases by pulmonary heart disease, 4 cases by pulmonary tuberculosis, 3 cases by liver cirrhosis and liver cancer with pulmonary metastases, and 2 cases by cerebral hemorrhage.</p><p><b>CONCLUSION</b>Among patients with CWP, the pathological changes of lung tissue become more complex with increasing years of dust exposure. Coal macule is the common pathological feature of CWP, and dust nodules and massive fibrosis are the necessary indices of pathological diagnosis.</p>

<p><b>OBJECTIVE</b>To explore the pathological changes of pulmonary fibrosis induced by SiO2 in rats and pigs.</p><p><b>METHODS</b>The silicosis models in rats and pigs were established by non-exposure method. The pathologic changes in lung tissues of rats and pigs were observed with HE staining under a light microscopy and under a transmission electron microscopy (TEM), the expression of cytokines was detected by immunohistochemistry.</p><p><b>RESULTS</b>(1) The main pathologic changes of silicosis models in rats and pigs included: in 7 ∼ 15 days after treatment, silica dusts, dust cells, a lot of macrophages, lung epithelial cells, a few neutrophils, macrophage alveolar inflammation and nodules of stage I were found in alveolar space; in 30 ∼ 90 days after treatment, many nodules of stage I-III or IV with lymphocytes infiltration were observed in respiratory bronchioles, alveoli, interlobular septa, the subpleural and around blood vessels and bronchi. (2) The expression levels of CK protein, SP-A protein, CD68, b-FGF, TNF-α, IL-6, TGF-β1, NFKappa/P50, Kappa/P65 and VEGF reduced with exposure time, but still were higher than those of the control. (3) The shed alveolar type I cells, proliferation of alveolar type II cells or macrophages and activated cellular function induced by silica were observed under TEM.</p><p><b>CONCLUSION</b>The development of pulmonary fibrosis in silicosis models corresponded with the process from macrophages alveolar inflammation to pulmonary fibrosis.</p>

<p><b>OBJECTIVE</b>To analyze the changes of semen quality in Chinese fertile men during the past 24 years.</p><p><b>METHODS</b>We reviewed the literature on the sperm quality of Chinese fertile males published from 1985 to 2008, collected relevant data, and analyzed the changes in semen volume, sperm density, and total sperm count by linear regression.</p><p><b>RESULTS</b>There were no significant changes either in semen volume during the period of 1985-2008 (P >0. 05) , or in sperm density and total sperm count from 1985 to 1994 (P > 0.05). However, significant decreases were observed during the latter 14 years in sperm density (from 81.5 x 10(6)/ml in 1995 to 66.7 x 10(6)/ml in 2008, with a mean annual reduction of 1.40%) and total sperm count (from 257.2 x 10(6) in 1995 to 185.9 x 10(6) in 2008, with a mean annual reduction of 2. 15%) (P < 0.05).</p><p><b>CONCLUSION</b>The semen quality of Chinese fertile men showed no significant changes from 1985 to 1995, but evidently declined from 1995 to 2008, which may be attributed to the aggravated environmental pollution induced by heavy chemical industry.</p>
